What is Nursing Home Transition?
The Nursing Home Transition program empowers and supports individuals 18-59 years of age who wish to transition out of a nursing home and return to living independently in the community. This program provides the services and resources an individual needs to make a smooth transition back into the community in order to experience a better quality of life.

If you answered yes, then JEVS Supports for Independence Nursing Home Transition is for you!
Services Include
- A personalized transition plan and support of a qualified coordinator
- Assistance in finding housing, planning your move, and setting up your household
- Assistance in finding resources to modify an existing residence
- Financial help with expenses, including security deposits, household items, adaptive equipment, and utilities
- Access to in home supports and community based services such as transportation, Personal Assistance Services, peer support, assistive technology, and home modifications
- Training in budget management
Eligibility
- Age 18-59
- Must be financially eligible
- Must currently reside in a nursing home in Bucks or Montgomery County Pennsylvania

JEVS Supports for Independence is dedicated to meeting your needs in a variety of languages. Our bilingual staff will create a supportive environment in which participants with limited English proficiency can have questions answered and their needs met.
