What is Home Choice?

The Home Choice program is open to Ohioans of any age who are living with disabilities and looking to transition away from long-term care facilities and into a home setting. This program provides individuals with greater choice and control over their services and quality of life, as well as assistance with moving into the community.

Home Choice Services Include
  • Assistance in moving back into the community including:
    • Locating a house
    • Setting up a household
    • Arranging and paying for goods and services necessary to live in the community such as health care, utility connection fees and household items
  • Transition Coordination to assist you with your transition plan
  • Financial Management Services to support you in managing your transition plan
What are Financial Management Services?
 

JEVS Supports for Independence Financial Management Services (FMS) support individuals enrolled in the Home Choice Program. Our services provide assistance to participants in managing and paying for services in their own customized transition plan. Our experienced staff coordinates payments to ensure that every participant’s transition is smooth and seamless.

Financial Management Services Include
  • Assistance with purchasing necessary goods and services which may include respite care, habilitative services, life coaching, nutritional services, home goods and home modifications
  • Tracking and reporting of individual participant budget activity
  • Payment to vendors and providers of approved services
  • Providing monthly and quarterly reports to participants and partnering agencies
Eligibility
  • Ohio residents of any age with disabilities
  • Currently residing in a long-term care facility for at least 90 days
  • Must be Medicaid eligible
