Home Choice is a unique program designed to support individuals in their desire to live satisfying and fulfilling lives at home.
Home Choice
The Home Choice program is open to Ohioans of any age who are living with disabilities and looking to transition away from long-term care facilities and into a home setting. This program provides individuals with greater choice and control over their services and quality of life as well as assistance with moving into the community.

- Greater choice and control over the services participants receive in their preferred setting
- Assistance in moving back into the community; some areas of assistance include:
- Locating a house
- Setting up a household
- Connecting to services and goods necessary to live in the community such as health care, pharmaceutical, and personal assistance
- Eligibility

- Open to Ohioans of any age who are living with disabilities and currently residing in a long-term care facility
- Must be Medicare or Medicaid eligible
