- What is the Ohio HOME Choice Program?
Ohio HOME Choice assists older adults and persons with disabilities to move from long-term services and support systems to home and community-based settings. HOME Choice provides participants with greater choice and control over the services received in their preferred setting and an Ohio HOME Choice Program transition coordinator to help them move back into the community.
Some areas of assistance include:
- Locating housing
- Setting up a household
- Connecting to community services
- Who is eligible to participate?

To participate in the Ohio HOME Choice Program, you must:
- Have lived in a facility-based care setting for at least six months,
- Have certain care needs, and
- Be eligible for Medicaid
- What is JEVS Role verses ODJFS' role?

- Determine eligibility
- Assign Case Manager
- Assist Consumer in choosing a transition coordination provider
- Authorize demonstration and supplemental service plan
- Authorize the payment of Transition Coordination deliverables
- Assist Consumer with securing service providers
- Fiscal Agent
- Manage the budgets of individual participants of the Ohio HOME Choice Program
- Provide monthly and quarterly reports to participants
- Process and pay pre-approved goods and services for participants
- Pay providers for demonstration services
- How do I become a provider?
- ODJFS approves provider applications, not JEVS. Provider applications can be found on ODJFS' website. Follow the icon on the right hand side titled "Visit the HOME Choice Provider site."
- Will I include my Medicaid number on claim forms submitted to JEVS?
- No. You will be assigned a separate and unique Ohio HOME Choice Program provider identification number.
- How much will the Ohio HOME Choice Program providers be reimbursed for services?
- Each Ohio HOME Choice Program service has a different reimbursement rate. Reimbursement rates are listed on the Provider Fact Sheets at http://jfs.ohio.gov/ohp/consumers/homechoice.stm. Look for the icon the right hand side titled "Visit the HOME Choice Provider site."
- How do I know that I am authorized to provide services to a consumer?
- If a consumer selects you to provide one of the HOME Choice demonstration services, the consumer's case manager will notify you and will authorize you on the HOME choice demonstration service plan.
- How do I submit a claim?
If you are a transition coordination provider submitting a claim for one of the three deliverables, then you need to complete the "Transition services Coordination Statement" in its entirety and submit the completed form to ODJFS. Upon approving the claim, ODJFS will submit the claim to JEVS to be paid. Once JEVS receives the error free claim form, you will be reimbursed within ten days.
For all other providers, you must submit your claim using the supplied Ohio HOME Choice Program claim form. The completed and error free form should be submitted directly to JEVS. Once JEVS receives the error free claim form, you will be reimbursed within ten days.
- If I need to contact JEVS about a reimbursement issue, whom should I call?
