Access Success Project

The Ohio Access Success Project (Success Project) expands Ohio's capacity to serve residents living in a nursing facility who want to live in a community setting and access services and supports to enhance their quality of life and independence. Through this service, recipients are provided with assistance in making relocation plans; accessing services including housing, transportation, and financial assistance; and one-time funding to support the transitional process.

Services
  • Assistance with arranging relocation
  • Assistance linking to needed services and supports — housing, transportation, financial assistance programs, and supportive services
  • One-time funding to assist with relocation expenses
Eligibility
  • Medicaid eligible nursing home residents with care needs that are no more than 80% of the average nursing facility
  • Eligibility confirmed Medicaid status and obtaining information from the facility staff
Enrollment

Referrals can be made by a nursing facility resident/staff member, family member, friend, relative, a Center for Independent Living, or Area Agency on Aging. For more information, call (614) 466-6742.

Languages Available

We have dedicated bilingual staff that can assist with the language needs of many individuals. We also have access to translation services. Please call for more information.
